Abstract
正常膝关节存在一定内翻角度,运动学与功能对线策略允许术后膝关节保留适度内翻。然而,相同的整体内翻可由不同股骨与胫骨内翻组合构成,其对假体受载与韧带应力的影响尚不明确。本研究旨在探讨不同股骨-胫骨内翻组合对全膝关节置换(TKA)术后生物力学表现的影响。采用几何建模与有限元分析相结合,构建不同内翻组合的全膝关节置换模型,评估屈膝0°~90°过程中下肢力臂、聚乙烯垫块应力及韧带拉力变化。结果显示,在相同整体内翻角与屈膝角度下,股骨内翻占比较高可相对降低垫块最大应力及前外侧与内侧副韧带拉力。垫块应力变化更受屈膝角度影响:屈膝90°时应力为屈膝0°时的约2.48倍,而内翻12°较0°仅增加约14%。前外侧韧带复合体在屈膝中承受最大牵张,起重要稳定作用。该研究从生物力学视角揭示了内翻组成对假体及周围软组织受力分布的影响,提示术中截骨策略应综合考虑股骨与胫骨对线组合。.
