Abstract

OBJECTIVE: Esophageal cancer is the main cancer burden in China, which is difficult to prevent and control. This study provided a scientific data support for the prevention and control of esophageal cancer in China by analyzing the trend of esophageal cancer incidence rate and the age of onset. METHODS: Based on the continuous cancer incidence data from 22 cancer registries of China between 2000 and 2015, the incidence was collated and analyzed by Joinpoint software. Age-specific incidence and mean onset age from 2000 to 2015 were calculated by regions and gender. After standardizing the age structure of the population in 2000 and 2015, the age-specific incidence composition in 2000 and 2015 were analyzed by regions and gender. RESULTS: From 2000 to 2015, the incidence of esophageal cancer in 22 cancer registries in China showed a downward trend, with an average annual decrease of 1. 4% (95%CI: -2.2%, -0.7%, Z=-3.7, P<0.05). After standardization the average annual percentage change was -4.2%(95%CI: -4.6%, -3.7%, Z=-20.7, P<0.05). The incidence of esophageal cancer increased with the increase of age. From 2000 to 2015, the incidence of esophageal cancer in all age groups showed a downward trend, where the younger age group decreased more significantly than the older group. At the same time, the average onset age in urban (F=42.370, P<0.001), rural(F=59.453, P<0.001), male(F=62.521, P<0.001) and female(F=216.442, P<0.001) increased with the increase of years. Only the trend of urban group disappeared after population age structure standardization. The disease composition of people over 65 years old, in urban and rural, male and female also increased, among which rural (from 44.24% to 55.80%) and female (from 48.70% to 65. 36%)were the most obvious. CONCLUSION: The incidence of esophageal cancer is declining, and the onset shows aging trend.
